Syringomyelia is a serious neurological disease in which fluid-filled cavities or cysts – so-called syrinxes – form in the spinal cord. It particularly affects brachycephalic breeds, above all the Cavalier King Charles Spaniel. The disease causes intense pain, movement restrictions and a wide range of neurological symptoms. The tricky thing about it: it often creeps up over many years before it is even detected.

Causes of Syringomyelia

How does this happen? The condition arises from several factors – sometimes in combination, sometimes individually.

Skull malformation (Chiari-like malformation):

By far the most common cause is a Chiari-like malformation. In this case, the back of the skull is simply too small to properly accommodate the cerebellum and brainstem. The cerebellum is pushed downwards, causing the flow of cerebrospinal fluid (CSF) to stagnate. The resulting increase in pressure then promotes the formation of the syrinx in the spinal cord.

Genetic predisposition:

Syringomyelia is more common in certain breeds – genetic factors clearly play a role. In addition to the Cavalier King Charles Spaniel, the Brussels Griffon, the Pug and the Yorkshire Terrier are particularly affected.

Inflammation or injury:

Less commonly, the cause lies in a trauma, an inflammation or an infection of the spinal cord. These can also disrupt CSF flow and ultimately lead to cyst formation.

Symptoms of Syringomyelia in Dogs

The clinical picture is multifaceted and varies greatly – depending on how advanced the disease is and which areas of the spinal cord are affected.

Pain:

The defining symptom is severe, chronic pain, which affected dogs often seem to communicate through a so-called burning sensation. Typically, the focus is on the neck or shoulder area, but the pain can also radiate to other parts of the body. Touch, certain movements or simply wearing a collar can trigger or significantly worsen the discomfort.

Phantom scratching:

A very characteristic sign is so-called phantom scratching: The dog scratches at the air – often towards the neck or nape – without actually touching the skin. This is caused by pressure on the affected nerves.

Hypersensitivity (allodynia):

Some dogs react with extreme sensitivity to touch. Even gentle stroking or brushing can trigger intense pain responses – particularly on the neck, back or shoulders.

Movement disorders and coordination problems:

In advanced stages, motor problems increasingly occur: weakness or paralysis of the hind legs, a stiff gait or pronounced coordination disorders (ataxia) are then no longer uncommon.

Head posture and neck problems:

Many affected dogs hold their head in an unusual position – an unconscious attempt to avoid pain. They avoid upward and downward movements of the head or appear generally stiff in their posture.

Seizures and neurological deficits:

In severe cases, seizures, incontinence or further neurological disorders may occur – a sign that the spinal cord is already significantly compromised.

Diagnosis of Syringomyelia

A reliable diagnosis requires a thorough neurological examination, almost always supplemented by imaging techniques.

Clinical examination:

The vet carefully palpates for pain reactions, checks sensitivity in the neck and back area and tests reflexes. This provides initial, important clues – but is usually not sufficient on its own.

MRI scan (Magnetic Resonance Imaging):

The MRI is the key diagnostic tool. It provides detailed cross-sectional images of the brain and spinal cord, on which cysts (syrinx) become directly visible. At the same time, malformations such as the Chiari-like malformation can be reliably visualised.

X-ray and CT:

X-rays and CT scans can show structural abnormalities in the skull or spine – however, for diagnosing syringomyelia itself, they are significantly less informative than an MRI.

CSF analysis:

Occasionally, the vet takes a sample of cerebrospinal fluid to rule out inflammation or infection as the cause – as these can produce similar symptoms.

Treatment of Syringomyelia

The condition cannot currently be cured. The aim of treatment is to alleviate symptoms and maintain quality of life for as long as possible. Depending on the severity, medication and surgical options are available.

Pain Management

Since pain is the central issue, effective pain management takes top priority. Medications used include:

  • Non-steroidal anti-inflammatory drugs (NSAIDs) – to reduce inflammation and pain.
  • Gabapentin or Pregabalin – particularly effective against neuropathic pain (nerve pain).
  • Corticosteroids – they can reduce inflammation and swelling in the spinal cord.
  • Opiates – in severe cases, when other remedies are insufficient.

Decompression

If the pressure on the spinal cord becomes too great, surgical intervention may be advisable: so-called foramen magnum decompression. This involves removing part of the skull bone to give the brain and brainstem more space. The operation alleviates symptoms and can slow the progression of the disease – however, it does not result in a cure.

Reduction of Cerebrospinal Fluid Production

Certain medications such as Omeprazole or Acetazolamide can reduce the production of cerebrospinal fluid. This lowers pressure in the system and brings noticeable relief to many dogs.

Physiotherapy and Supportive Care

Often underestimated, but valuable: physiotherapy helps maintain mobility and makes pain more manageable. Practical measures in everyday life – such as using a soft harness instead of a collar or consciously avoiding physically strenuous activities – can noticeably reduce pressure on the neck.

Prognosis and Quality of Life

It is impossible to make a general statement about how an affected dog fares in the long term – this depends very much on the severity and on how well it responds to treatment. Some dogs live for years with drug therapy and maintain an acceptable quality of life. Others, particularly in severe cases, experience a progressive deterioration of neurological functions despite everything. Surgical treatment can improve the situation, but it often does not permanently halt the progression of the disease.

The crucial point is: an early diagnosis makes a real difference. The earlier the correct treatment begins, the better the quality of life can be preserved. Regular veterinary check-ups and consistent adjustment of medication remain indispensable in the long term.
