Myopathy refers to a group of diseases that directly affect the skeletal musculature of dogs and lead to muscle weakness, movement disorders or muscle wasting.

Myopathy – this sounds abstract at first, but it hits a very concrete point: the dog’s own skeletal musculature. Not the nerves, not the brain – but the tissue that drives every step, every jump, every greeting. Affected dogs struggle with muscle weakness, movement problems or creeping muscle wasting.
What distinguishes myopathy from neurological diseases is an often overlooked detail: The nerve pathways function. The reflexes are there, everything as usual – and yet the strength collapses. The error lies deeper, in the muscle fibres themselves. This makes the diagnosis so tricky because the picture looks little dramatic at first glance.
What forms of myopathy exist in dogs?
Four main forms shape the clinical picture – and they have little in common except for the result: weakened musculature.
Hereditary myopathy: Here the error lies in the genetic code. Labrador Retrievers are particularly known for centronuclear myopathy, Great Danes struggle with dystrophin deficiency, Cavalier King Charles Spaniels also have a genetic susceptibility. In Labradors, weakness often shows up between the third and sixth month of life – so right in the puppy stage, when one would not expect anything like this.
Inflammatory myopathy: Polymyositis is the best-known variant. It attacks symmetrically – masticatory muscles and limbs equally. Dogs chew laboriously, their gait appears stiff and somehow braked.
Metabolic myopathy: Hypothyroidism leaves a characteristic picture: The musculature feels firm and swollen, but reacts weakly to stimuli. This sounds contradictory – and it is somewhat so.
Toxic myopathy: Medications such as colchicine or statins can attack muscle fibres directly. This happens not infrequently when dogs accidentally get access to human medicines – a moment of carelessness can be enough.
How do I recognise myopathy in my dog?
The tricky thing: The symptoms come quietly. Many owners attribute them to old age or the last long outing for months.
Movement abnormalities: The dog gets tired quickly – after distances that were not an issue before. It pants earlier, seeks breaks, sits down in the middle of the path. Whoever knows their dog well notices the difference. Whoever does not know them so well might think: it was a hot day.
Muscle changes: In hereditary forms, the body posture changes – the limbs appear excessively bent, the musculature softer than it should be. This is hard to grasp if one has no direct comparison.
Swallowing difficulties: Particularly with polymyositis, it becomes apparent that dogs chew more slowly or lose food from their mouth. No choking, no drama – simply a quiet failure of the oral apparatus when eating.
Stiff movements: In the morning or after long rest periods, the dog moves as if rusted. After a few minutes of movement, it loosens up again – which makes the matter even harder to classify because it then feels almost normal.
How does the vet diagnose myopathy?
The diagnosis is not a single test, but a staged process. Whoever expects a clear answer straight away will be surprised.
Creatine kinase test: Elevated CK levels in the blood are an initial indicator of muscle cell damage. Levels above 200 U/l are considered suspicious, and above 1000 U/l as significantly elevated. Important: The test must be carried out on an empty stomach and without prior exercise – even a thorough walk can skew the results.
Electromyography: EMG measures the electrical activity of the muscles and helps distinguish between primary muscle problems and nerve damage. In myopathy, typical patterns of spontaneous electrical discharges appear – virtually a fingerprint of the disease.
Muscle biopsy: A tissue sample from the M. vastus lateralis – the outer thigh muscle – provides information on signs of inflammation, fibre degeneration or structural changes. This is invasive, but often the only way to gain real clarity.
Genetic tests: For breeds with known hereditary myopathy, specific DNA tests exist. They can detect a predisposition before the first symptom appears – which is a genuine advantage for breeders and proactive owners.
What treatment options are available?
The therapy always follows the cause. There is no universal solution – but depending on the form, there are certainly effective approaches.
Drug therapy: In inflammatory forms, corticosteroid preparations such as prednisolone or immunosuppressants such as azathioprine are used. The dosage is determined individually by the vet practice and reduced step-by-step over weeks – stopping too quickly means risking a relapse.
Physiotherapy: Controlled movement is not just a nice addition, but part of the treatment. Swimming has proven effective: it spares the joints and trains the muscles at the same time. Passive stretching exercises help prevent contractures – i.e. the freezing of muscles in incorrect positions.
Dietary adjustment: High-quality protein – approx. 25–30 % of dry matter – actively supports muscle maintenance. Coenzyme Q10 and vitamin E can reduce oxidative stress, which additionally burdens the muscle fibres. No miracle cure, but a sensible building block.
Activity management: Regular, moderately dosed exercise keeps the muscles functional. Intensive exertion, on the other hand, can cause further damage – finding the balance is often the real task for everyday life.
What is the prognosis for myopathy?
An honest answer: It depends. On the form, on the time of diagnosis, on the consistency of treatment.
Inflammatory myopathies often respond well to immunosuppression – around 70 % of affected dogs show significant improvement within 8 to 12 weeks. Metabolic forms – e.g. triggered by hypothyroidism – improve completely in many cases as soon as the underlying disease is treated.
Hereditary myopathies follow a progressive course. There is no cure. But with adapted living conditions, targeted physiotherapy and realistic expectations, quality of life can be maintained for years – this is not a defeat, but a genuine success.
Those who act early and stay consistent give their dog a real chance at a fulfilling life – with adapted activities, regular check-ups and an everyday routine that suits the disease rather than fighting against it.
