Total protein – sometimes also called total protein – is a lab value from the dog’s blood count that measures the total amount of all circulating proteins in the blood plasma. Specifically, this refers mainly to albumin and globulins, both of which perform very different but equally important tasks. Anyone who has looked more closely at their dog’s blood count has probably come across this value – and may have wondered what it actually indicates. In short: it provides clues about the health of the liver and kidneys, nutritional status and whether an inflammation or infection is currently brewing in the body.

What lies behind total protein?

Total protein consists of all proteins dissolved in the blood plasma. The two main pillars are:

  1. Albumin: Albumin makes up the lion’s share of total protein. It transports nutrients and hormones through the blood and – which is often underestimated – regulates fluid balance in the vessels, thereby keeping blood pressure stable.
  2. Globulins: This protein family is more diverse. It includes alpha-, beta- and gamma-globulins, which help with lipid and metal ion transport and primarily keep the immune system running. Gamma-globulins are, for example, antibodies – precisely what the body produces in increased amounts during an infection.

Why is the total protein value measured at all?

Because it reveals surprisingly much. A single blood value that can indicate liver problems, kidney failure, immune reactions or deficiencies – that is already remarkable. Changes in total protein levels can point to liver disease, kidney disease, inflammation, infections or immune system disorders.

Normal values in dogs

There is no universally valid normal range: the reference range is stated on your lab report and depends on the laboratory and measurement method. Only against this range can your dog’s value be classified. If the value lies outside this window, it is not a verdict but a clear signal for further investigations.

Causes of elevated total protein values

A value that is too high may sound less alarming at first glance than one that is too low – but it is just as serious. Possible causes:

  1. Dehydration: The most common cause, and often the least harmful. If a dog drinks too little or loses fluid (for example through vomiting or diarrhoea), the fluid proportion in the blood falls – the protein concentration appears to rise, even though the absolute amount remains the same.
  2. Chronic inflammation or infections: If an infection or inflammation persists for longer – for example due to bacterial or viral diseases –, the immune system produces increased amounts of antibodies. The globulin level climbs, and so does total protein.
  3. Autoimmune diseases: When the immune system attacks the body’s own structures, it responds with increased globulin production – a pattern that is reflected in the blood count.

Causes of low total protein values

A drop in total protein indicates that the body is lacking proteins – either because too little is being produced, too much is being lost, or both are happening at once:

  1. Liver diseases: In severe liver failure, the liver can no longer produce sufficient albumin. The total protein value falls – and so does the blood’s ability to retain fluid in the vessels.
  2. Internal or external blood loss and protein loss via the intestines are the two most common mechanisms behind a total protein that is too low. Whether additional protein is lost via the kidneys is clarified by a urine test.
  3. Gastrointestinal diseases: Chronic intestinal inflammation or intestinal lymphangiectasia – a condition in which the lymph vessels in the intestines are blocked – can lead to proteins being lost directly via the digestive tract.
  4. Blood loss: In cases of severe internal or external bleeding, not only red blood cells are lost, but also albumin and other proteins. After severe trauma, the total protein value can drop accordingly.

How is it measured?

The measurement is straightforward: The vet takes a blood sample, which is then analysed in the laboratory. Two methods are primarily used:

  1. Biochemical analysis: The blood chemistry panel is the standard procedure. It provides not only the total protein value, but also albumin, globulins, as well as liver and kidney values – all in one go.
  2. Refractometry: Faster, but less detailed: A refractometer uses light refraction to determine the protein content of a fluid. Practical for an initial assessment directly in the practice.

What to do if the value deviates?

A deviating total protein value is initially an indication, not a verdict. The next step is always to search for the cause. Depending on the suspicion, the vet will initiate further examinations, such as:

  • Urine analysis, to prove or rule out protein loss via the kidneys.
  • Ultrasound or X-ray, to make inflammation foci, changes in organs or tumours visible.
  • Blood coagulation tests, which allow conclusions about liver function.
  • Special tests for autoimmune diseases or cancers such as multiple myeloma.

Treatment then depends entirely on what lies behind the value. In the case of simple dehydration, a fluid therapy is often sufficient to bring total protein back into the normal range. In the case of liver or kidney diseases, however, well-considered treatment plans tailored to the individual dog are needed – and usually also regular control blood counts to keep an eye on the course.
