Cystocentesis – this sounds like a complicated procedure at first, but it is a relatively quick routine measure in everyday veterinary practice. The vet inserts a fine needle through the dog’s abdominal wall directly into the urinary bladder to collect urine. Although it may sound uncomfortable, it serves a clear purpose: to obtain a sterile urine sample that is not contaminated by bacteria from the environment or the lower urinary tract. This is a decisive advantage, particularly when investigating urinary tract infections, cystitis, or other urinary tract diseases.

When is cystocentesis performed?

This method is used whenever a standard urine sample is simply not precise enough. This happens more often than you might think:

  1. Suspected urinary tract infections: If the dog urinates unusually frequently, shows pain when urinating, or has bloody urine, a sample that is truly informative is required. Cystocentesis collects urine directly from the bladder – contamination via the urethra is thus excluded.
  2. Bladder stones (urolithiasis): If there is suspicion of bladder stones or urinary crystals, the exact composition of the deposits can be determined from the sample obtained via cystocentesis – forming the basis for choosing the correct treatment.
  3. Investigation for tumours: If there is suspicion of neoplasms of the urinary bladder, the urine can be examined for abnormal cells or blood.
  4. Unclear urine loss: In dogs with incontinence or involuntary urine loss, cystocentesis can help to specifically identify underlying bladder problems.
  5. Sterile samples for a urine culture: When bacteria or fungi in the urinary tract are to be detected or ruled out, only a sample without external contamination counts – and this is exactly what this procedure provides.

How does cystocentesis work?

The procedure is minimally invasive and usually takes only a few minutes. Anaesthesia is almost never necessary. The typical process in practice looks like this:

  1. Positioning the dog: The dog is positioned on its back or in lateral recumbency. The abdominal area is cleaned to ensure a sterile environment.
  2. Locating the bladder: The vet palpates the urinary bladder either by palpation or uses ultrasound – particularly if the bladder is poorly filled or difficult to feel.
  3. Puncture: A sterile needle is carefully guided through the abdominal wall into the bladder. The urine is withdrawn using a syringe.
  4. Sample collection: The urine is transferred into sterile tubes and sent to the laboratory – for analysis or for a bacterial culture.
  5. Aftercare: Most dogs show hardly any discomfort after the procedure and can go straight home. For more sensitive animals, the vet sometimes recommends a short observation period.

Why not just catch the urine?

A valid question. Compared to free catching or catheterisation, cystocentesis has several solid advantages:

  1. Truly sterile sample: Urine collected directly from the bladder contains no bacteria from the urethra or skin – this is the key point. All other methods carry this risk of contamination.
  2. Higher diagnostic certainty: Because nothing from the outside can influence the result, the accuracy of diagnosis for urinary tract infections or bacterial cultures is significantly better.
  3. Quick and practical: No complex setup, usually no sedation – the procedure fits seamlessly into everyday practice routine.
  4. Good tolerance: With an experienced vet, the risk of complications is low; most dogs tolerate the procedure well.

What are the risks?

Cystocentesis is considered a safe procedure; nevertheless, you should be aware of the possible risks – even though they occur rarely:

  1. Minor bleeding: Occasionally, minor bleeding may occur at the puncture site or in the bladder. This usually stops on its own and is clinically insignificant.
  2. Bladder perforation: In rare cases, there is a risk of accidentally piercing the bladder wall. Correct technique and the use of ultrasound significantly reduce this danger.
  3. Infection: Although the procedure is intended for sterile sample collection, a low risk of infection can never be completely ruled out – particularly if sterility of the environment or materials is not ensured consistently.
  4. Injury to adjacent structures: If the needle is not placed correctly, surrounding organs such as the intestine, blood vessels, or – in bitches – the uterus could be affected. However, this is extremely rare in experienced hands.

What alternatives are there?

Depending on the situation and the question at hand, other methods of urine collection may also be considered:

  • Catch method: The simplest option – urine is collected in a clean container during spontaneous urination. Practical, but often insufficient for precise diagnostic purposes because the sample can easily become contaminated by bacteria from the urethra or skin.
  • Catheterisation: A urinary catheter is inserted into the urethra to collect urine directly from the bladder. Technically simpler in males; in bitches, the procedure is more involved and may require mild sedation. A certain risk of infection remains here as well.

Assessment

Cystocentesis is an established, safe and diagnostically precise procedure for obtaining a reliable urine sample from a dog – particularly when there is suspicion of urinary tract infections or bladder problems. When performed by an experienced vet, the risk of complications is minimal, and the diagnostic yield is significantly higher compared to other collection methods.
