This happens when the kidneys can no longer fulfil their primary function: filtering waste products out of the blood and eliminating them via urine. Uraemia is usually considered a sign of advanced kidney disease and can become life-threatening if the dog is not treated in time.
What is uraemia?
The term uraemia literally means “urine in the blood” – and that is exactly what it describes: normally, healthy kidneys continuously filter breakdown products such as urea (an end product of protein metabolism) and creatinine out of the blood. If the kidneys fail partially or completely, these substances remain in the bloodstream and accumulate to toxic levels.
Uraemia is not an independent disease picture, but a symptom of kidney insufficiency or kidney failure. It typically occurs in the late stage of chronic kidney disease (CKD) – or very suddenly in acute kidney failure (AKI).
Causes of uraemia in dogs
Uraemia is almost always the result of kidney failure or severely reduced kidney function. An overview of the main triggers:
Chronic Kidney Disease (CKD)
CKD develops insidiously – often over months or years. Many dogs show no symptoms for a long time until kidney function has dropped to around 25% or less of normal capacity. Common causes:
- Age – older dogs are significantly more frequently affected
- Genetic predisposition
- Infections, e.g. leptospirosis
- Long-term use of kidney-damaging medications, such as NSAIDs
- Kidney tumours or polycystic kidney disease
Acute Kidney Failure (AKI)
AKI sets in without warning. Within hours or days, kidney function can collapse dramatically. Possible triggers:
- Poisoning – for example by antifreeze, grapes or certain medications
- Heatstroke or severe fluid loss
- Infections such as leptospirosis or pyelonephritis
- Shock states or drop in blood pressure
Obstructive Uropathies
Blockages in the urinary tract – for example due to urinary stones or tumours – cause urine flow to back up. This backflow damages the kidneys and slows the excretion of waste products.
Kidney Inflammation (Nephritis)
Inflammatory processes such as glomerulonephritis directly attack the filtering function of the kidneys and can thus pave the way for uraemia.
Symptoms of uraemia in dogs
The tricky thing about uraemia: the symptoms are varied and initially appear non-specific. However, anyone who knows their dog well often notices early on that something is wrong. Essentially, the complaints arise because waste products and excess fluid accumulate in the body and disrupt normal bodily functions.
General symptoms:
- Lethargy and noticeable tiredness
- Weakness and lack of interest in activities or play
- Increased drinking and frequent urination (polydipsia and polyuria)
- Loss of appetite
- Nausea and vomiting
- Bad breath with a characteristic ammonia odour – dog owners often describe it as “stale” or “chemical”
Gastrointestinal symptoms:
- Vomiting and diarrhoea
- Abdominal pain and bloating
- Ulcers in the mouth and on the tongue
Neurological symptoms:
- Muscle twitches or cramps
- Staggering and uncoordinated running (ataxia)
- In severe cases: coma or sudden collapse
Cardiovascular symptoms:
- High blood pressure (hypertension), which in turn can predispose to heart problems
- Oedema – fluid retention in the legs or abdominal cavity
Signs of dehydration:
- Dry mucous membranes
- Sunken eyes
- Weak pulse and general weakness
Diagnosis of uraemia in dogs
To reliably establish uraemia, a thorough clinical examination as well as blood and urine tests are required. Only in this way can the condition of the kidneys be assessed realistically and the severity of the disease evaluated.
Blood tests:
- Blood urea nitrogen (BUN) and creatinine are the most important markers for kidney function. In dogs with uraemia, both values are typically significantly elevated.
- Electrolytes such as potassium, sodium and phosphate are also monitored, because kidney failure can severely disrupt their levels.
Urine tests:
- A urine analysis shows how concentrated the urine still is. Dogs with kidney failure often excrete highly diluted urine.
- Detection of protein, blood or bacteria in the urine can indicate the actual cause of the kidney disease.
Imaging techniques:
- Ultrasound or X-rays of the kidneys can reveal structural changes – such as kidney tumours, kidney stones or abnormal size changes.
- Sometimes an additional kidney biopsy is necessary to narrow down the cause of the disease more precisely.
Treatment of uraemia in dogs
The treatment pursues three objectives: to address the cause of kidney failure, to alleviate symptoms and to stabilise or support kidney function as far as possible. How the therapy looks in concrete terms depends on the severity of the uraemia and the underlying cause.
- Fluid therapy: An intravenous infusion is almost always at the beginning – it balances fluid levels, flushes toxins from the body and stabilises blood pressure. This measurably relieves the kidneys and simultaneously combats dehydration.
- Diuretics: Sometimes diuretic agents are used to stimulate urine production and excrete waste products more quickly.
- Dietary adjustment: A special kidney-friendly diet – reduced protein and phosphorus content, but with high-quality protein sources and balanced electrolytes – can noticeably reduce the burden on the kidneys and lower blood urea levels.
- Medications for the underlying disease:
- Antibiotics for kidney infections such as pyelonephritis or leptospirosis
- Blood pressure-lowering agents, to relieve the kidneys
- Phosphate binders, which keep an elevated phosphate level – common in kidney failure – under control
- Dialysis: In cases of severely impaired kidney function, dialysis may become necessary to mechanically cleanse the blood of toxins. This treatment is complex and remains reserved for specialised veterinary clinics.
- Emergency treatment for poisoning: If poisoning has triggered uraemia, the dog immediately receives antidotes and targeted detoxification therapies to remove the poison from the body as quickly as possible.
Prognosis
How the outlook for a dog with uraemia looks depends largely on how advanced the kidney damage is, how early the diagnosis was made and how well the dog responds to treatment. Acute kidney failure can – if recognised quickly and treated consistently – actually be reversed in some cases. Chronic kidney diseases, on the other hand, are not curable: The objective here is to slow down progression and maintain the best possible quality of life for the dog.
Conclusion
Uraemia is not a condition where you should wait and see. It arises when the kidneys fail so severely that toxic metabolic products climb to dangerous levels in the blood – and it can deteriorate dramatically in a short time. The earlier a vet is involved, the greater the chance of stopping or at least slowing down the process.
