Hypokalaemia
Hypokalaemia is a condition in which the potassium level in a dog's blood falls below the normal reference range of 3.5 mEq/L and can seriously impair muscles, nerves and the heart.

Hypokalaemia – this is what it is called when the potassium level in a dog’s blood drops below the normal value. It may sound abstract at first, but it is anything but harmless: potassium is a vital electrolyte without which cells, muscles, and nerves simply do not function properly. If levels drop too far, the heart, musculature, and entire nervous system can be seriously affected. Triggers can be diseases, certain medications, or simply unbalanced nutrition.
What is hypokalaemia – and when does it become critical?
In short: hypokalaemia occurs when the potassium level has fallen below the normal reference range. Potassium keeps the body’s fluid and electrolyte balance in check – the heart muscle relies on it in particular. If the value drops too much, muscles and nerves fail: muscle weakness, arrhythmias, and other serious problems can follow.
In a healthy dog, the potassium level is between 3.5 and 5.5 mEq/L (milliequivalents per litre). If it falls below 3.5 mEq/L, this is termed hypokalaemia – and veterinary treatment is usually necessary.
How does hypokalaemia develop in dogs?
The causes are varied. Here are the most common ones:
Kidney diseases:
- Chronic kidney disease: Dogs with kidney failure often excrete too much potassium in the urine because the kidneys can no longer retain it sufficiently.
- Acute kidney failure: Sudden kidney damage or acute kidney failure can also disrupt potassium balance.
Vomiting and diarrhoea:
- Anyone who has cared for a dog with multi-day diarrhoea knows: the body loses not only water but also electrolytes – including potassium. In prolonged or severe cases, this can lead to pronounced hypokalaemia.
Hormonal diseases:
- Hyperaldosteronism: In this rare disease, the adrenal glands produce too much aldosterone – a hormone that increases potassium loss through urine.
- Diabetes mellitus: Dogs with poorly controlled diabetes also lose increased amounts of potassium through urine.
Medications:
- Diuretics (water pills): These agents are frequently used for heart or kidney diseases and flush fluid and electrolytes – including potassium – out of the body.
- Insulin: A dose of insulin that is too high can lower potassium levels because insulin promotes the transport of potassium from the blood into the cells.
Malnutrition:
- Dogs that take in too little potassium through food over a long period also risk hypokalaemia – although this is relatively rare in practice.
Fluid therapy:
- Intravenous infusions without sufficient potassium can contribute to potassium loss with prolonged use – especially in dogs that already have kidney or heart problems.
How do you recognise hypokalaemia in a dog?
The symptoms depend on how much the potassium level has dropped:
- Muscle weakness: The dog appears limp, struggles to stand up or walk, muscles tremble or seem barely bearable. In severe cases, complete paralysis can occur.
- Weakness and lethargy: Reduced activity, quicker exhaustion, sometimes also refusal of food – these are typical but non-specific warning signs.
- Constipation: Hypokalaemia can weaken the intestinal muscles and slow down bowel movement – with constipation as a result.
- Increased urination and thirst: Particularly in dogs with kidney disease or diabetes, hypokalaemia can lead to increased urine output (polyuria) and heightened thirst (polydipsia).
- Heart problems: A low potassium level can trigger irregular heartbeats – cardiac arrhythmias, which in the worst case can become life-threatening.
- Breathing difficulties: In severe cases, hypokalaemia also weakens the respiratory muscles, which can lead to shortness of breath.
How is hypokalaemia diagnosed?
A veterinary examination and targeted tests are required for diagnosis:
- Blood tests: The potassium level is measured directly from the blood – a low value confirms the diagnosis.
- Electrolyte analysis: Sodium, calcium and chloride are usually determined at the same time to obtain a complete picture of the electrolyte status.
- Urine tests: A urine analysis can show whether the dog is excreting too much potassium via the urine – an indication of a kidney or hormonal disorder.
- ECG (electrocardiogram): If there is suspicion of cardiac arrhythmias, an ECG is performed to assess electrical heart activity.
- Imaging techniques: Ultrasound or X-rays help to detect possible underlying conditions – such as kidney or liver problems.
Treatment: What can be done?
Treatment depends on the severity of potassium loss and the cause behind it. The goal: normalise potassium levels and address the trigger.
Potassium supplementation:
- Oral potassium preparations: In mild cases, oral preparations or potassium-rich foods are often sufficient to raise the level again.
- Intravenous potassium therapy: If the dog shows muscle weakness or heart problems, potassium via infusion is required. Caution is advised here: correction that is too rapid can be dangerous; the infusion must be closely monitored.
Treatment of the underlying condition:
- Kidney disease: Special diets and medications that support kidney function and regulate electrolyte balance.
- Hormonal disorders: Dogs with diabetes or hyperaldosteronism require specific therapies – such as insulin or hormone treatments – so that the underlying condition comes under control and hypokalaemia improves.
- Vomiting and diarrhoea: Here, the aim is to stop fluid and electrolyte loss – often with intravenous infusions and an adapted diet.
Fluid therapy: For severely dehydrated dogs or those with kidney problems, targeted fluid therapy helps restore balance and stabilise potassium levels.
Heart monitoring:: Dogs with severe hypokalaemia and cardiac arrhythmias require intensive monitoring. If necessary, antiarrhythmics are used to prevent dangerous heart complications.
Prognosis
How well a dog recovers depends largely on the cause and extent of potassium loss. If the underlying condition is detected and treated quickly, many dogs recover completely. With chronic problems such as kidney failure or hormonal disorders, long-term treatment may be necessary to keep potassium levels within the normal range over the long term.
Prevention: What can be done?
Hypokalaemia cannot always be prevented entirely – but with a few measures, the risk can be significantly reduced:
- Regular vet visits: Routine examinations help detect kidney problems or diabetes at an early stage before they lead to electrolyte disorders.
- Keep medication in view: Dogs receiving diuretics or insulin should be checked regularly – so that their potassium level does not drop unnoticed.
- Balanced nutrition: A potassium-rich, wholesome diet helps keep the electrolyte balance stable.
