How is fentanyl administered?

Depending on the situation and clinical setting, three forms are available:

  1. Intravenous injection: The most direct route – acts quickly, therefore preferred in emergencies or immediately in the recovery room after surgery.
  2. Fentanyl patch: The patch is applied to a shaved area of skin and releases the active ingredient evenly over several days. For owners caring for a dog at home, this is a practical option – provided the patch is applied correctly and not licked off or removed by the dog.
  3. Continuous infusion: In the clinic, fentanyl can be given as a continuous intravenous infusion. This allows the team to adjust the dose precisely and monitor the dog continuously.

How does fentanyl work in the dog’s body?

Fentanyl binds to opioid receptors in the brain and spinal cord, thereby interrupting the transmission of pain signals. The effect sets in quickly and is highly reliable for severe pain. Many dogs appear calmer and more relaxed after administration – this is a direct result of dampened pain perception, not a separate sedative effect.

Side effects and risks

The strength of the drug comes at a cost. Because fentanyl acts deeply on the nervous system, it can affect several body systems simultaneously. The most important side effects:

  2. Sedation: Drowsiness and lethargy are common. However, a very deeply sedated dog may also indicate an overdose.

  4. Nausea and vomiting: Not in every dog, but frequent enough to watch out for it.
  5. Constipation: Opioids slow down bowel activity. With longer-term use, this can become a problem.
  6. Tolerance and dependence: With prolonged use, tolerance may develop – meaning the original dose eventually becomes insufficient. Physical dependence is also possible, which is why discontinuation should be done gradually.

Contraindications

Fentanyl is not suitable for every dog. Special caution is advised with certain pre-existing conditions:

  1. Respiratory diseases: Dogs with lung diseases or impaired respiratory function carry an increased risk of respiratory depression. If used at all, only under intensive monitoring.
  2. Liver or kidney problems: A weakened metabolism can lead to fentanyl building up in the body instead of being broken down – with correspondingly intensified effects.
  3. Pregnancy and lactation: In pregnant or nursing bitches, the active ingredient can pass to the unborn or newborn animal. Use only in truly justified exceptional cases.

Monitoring during treatment

Fentanyl demands control – this is no exaggeration, but simply the consequence of its potency. In Switzerland, veterinarians monitor the following parameters during treatment:

  • Respiratory rate: Checked regularly because respiratory depression is the most dangerous side effect.
  • Heart rate and circulation: Bradycardias can creep in, which is why continuous monitoring is important.
  • Level of sedation: A dog that is too deeply sedated signals that the dosage needs to be reconsidered.
